Healthcare Respiratory Protection: Building and Running a PAPR Program
A hospital respiratory protection program is not a single purchase. It is a platform that has to be assembled, powered, verified before use, kept comfortable across twelve-hour shifts, and reconfigured when the threat changes. ILC Dover supplies the complete Sentinel XL systems, head tops, cartridges and components that make that possible, with conversion kits that take the same platform from airborne isolation to hazardous drug handling and decontamination.
What a Complete Sentinel Setup Needs
Sentinel is a component system rather than a sealed appliance, which is what allows one platform to serve several departments. A working configuration brings together:
-
1
A blower
The Sentinel XL Blower Assembly, a rugged dual chamber PAPR blower compatible with a range of head covers and hoods
-
2
A power source
Rechargeable NiMH or alkaline, depending on whether the unit is in daily rotation or held in reserve
-
3
Cartridges
HEPA P100 for particulate and biological protection, OV/AG/HE where organic vapors or acid gases are also present, or 40mm CBRN cartridges for decontamination configurations
-
4
A head top
Head cover, clear hood or BioShield full hood for HP configurations, or the butyl hood for CBRN, selected against the exposure
-
5
A breathing tube
An extendable breathing tube connecting the blower to the head top
Because the interfaces are shared, a department can change its level of protection by changing the head top and cartridge rather than replacing equipment. The table below matches each configuration to the clinical task.
Matching the Sentinel Configuration to the Clinical Task
Each row pairs a head top with a cartridge and lists the ready-made system or conversion kit that supplies it. Detailed hood guidance and protection factors are set out on the infectious disease page.
| Clinical task | Head top | Cartridge | Ready-made options |
|---|---|---|---|
| Routine airborne isolation | Head cover, APF 25 (3-pack or 25-pack) | HEPA P100 (3×2 or 25×2) | HP Standard System (S-3000-01), Head Cover Conversion Kit (S-3000-05) |
| Tasks needing a wide field of view | Clear hood, APF 1,000 (3-pack or 30-pack) | HEPA P100 | HP Clear Hood System (S-3000-16), Clear Hood Conversion Kit (S-3000-19) |
| High-consequence pathogens with splash or contact risk | BioShield full hood, APF 1,000, taped seams (3-pack or 25-pack) | HEPA P100 | HP BioShield Full Hood System (S-3000-23), BioShield Conversion Kit (S-3000-24) |
| Hazardous drug handling (USP 800) | BioShield full hood, APF 1,000 | OV/AG/HE | BioShield HD Conversion Kit (S-3000-31) |
| Hospital decontamination | CBRN butyl hood | CBRN Cap 2 | CBRN Conversion Kit (S-5000-05), or a complete CBRN system (see below) |
Ready-to-Use Systems or a Base Kit Plus Conversion Kits
There are two ways to equip a department. A complete system arrives ready to use in one configuration. A base kit supplies the durable hardware once, and conversion kits add the head top, breathing tube and cartridges for each configuration the department needs.
-
Sentinel XL HP Standard System
SKU S-3000-01
Head cover and HEPA cartridges with the blower, NiMH battery, Smart Battery Charger, flow meter, breathing tube, Quick-Loc waist belt and duffel bag.
-
Sentinel XL HP Clear Hood System
SKU S-3000-16
Clear hood and HEPA cartridges with an alkaline battery pack that runs on standard D-cells, so no charging is required.
-
Sentinel XL HP BioShield Full Hood System
SKU S-3000-23
BioShield taped full hood and HEPA cartridges with alkaline power. The hood passes ASTM F1670-14 and ASTM F1671-13 for synthetic blood and bloodborne pathogen penetration.
-
SKU S-3000-12
Blower, NiMH battery, Smart Battery Charger, flow meter, breathing tube, belt and duffel bag, with no hood or cartridges, so each unit can be finished with the conversion kit a department needs.
Conversion kits are available for the head cover, clear hood, BioShield full hood, hazardous drug and CBRN configurations. A department that buys base kits once can move between configurations by buying a kit rather than a new system.
Hazardous Drug Handling and Disinfection
HEPA cartridges are designed for particulates. When a task also involves organic vapors or acid gases, as with hazardous drug handling or sporicidal disinfection, the same Sentinel XL blower and batteries are used with a different hood and cartridge.
-
BioShield Full Hood HD Conversion Kit (USP 800)
SKU S-3000-31
BioShield full hood, breathing tube and OV/AG/HE cartridges that configure a compatible Sentinel XL system for hazardous drug protection. APF 1,000 with no fit testing required.
-
Sentinel XL OV/AG/HE Cartridges (Pack of 3)
SKU S-4012
NIOSH-approved protection against organic vapors and acid gases with built-in HE filtration at 99.97% efficiency. ILC Dover has tested it against sporicidal disinfectants based on peracetic acid and hydrogen peroxide, which suits teams that disinfect healthcare facilities. One pack is a full set for the blower.
Change-out note
OSHA requires gas and vapor cartridges to be replaced on a change-out schedule, and the S-4012 technical bulletin on the product page gives breakthrough times to help set one. The S-4012 is not intended for ammonia or formaldehyde, so contact ILC Dover before specifying it for pathology or laboratory areas that use formaldehyde.
Powering a PAPR Fleet Across Shifts
Battery strategy is where PAPR programs usually succeed or struggle, because a unit with a flat battery is functionally absent from the inventory.
Sentinel XL Rechargeable NiMH Battery Pack
SKU S-2003
Up to 8 h
Run time per charge
Suited to units in daily clinical rotation.
Sentinel XL Alkaline Battery Pack
SKU S-4013
Up to 10 h
Run time on five D-cells
An adapter supplied with five Duracell D-cell batteries. Shelf stable, so readiness does not depend on charge state.
-
Sentinel XL Smart Battery Charger
SKU S-2009
Interfaces with the NiMH pack, and the battery can stay in the blower while charging, so units can be returned to a charging point without being stripped down.
Fleet tip
Many hospitals hold both chemistries: rechargeable packs for the working fleet, alkaline packs for surge units and emergency reserve where a charging cycle cannot be assumed.
Verifying Airflow Before Use
Manufacturer airflow checks are a standard element of PAPR use, and they need a device to perform. The Sentinel Flow Meter (S-2010) is an easy to read meter compatible with Sentinel XL and XT blowers, durable enough for repeated daily use. One is included in every complete Sentinel system and base kit on this page, and spares let each department run its checks without waiting for a shared meter.
Comfort and Wearability Across a Full Shift
Protection that staff dread wearing tends to be worn incorrectly or not at all. Two components do most of the work here:
-
SKU S-3508
Extends from 19 inches compressed to 48 inches fully extended, one size for all wearers. Made from thermoplastic polyurethane so it flexes with movement, lays flat, and is latex free and hypoallergenic.
-
SKU S-4011
A click-on belt for Sentinel XL and XT systems, in polymeric construction that is easy to clean between users.
Latex-free construction and cleanable surfaces matter more in healthcare than in most settings, because equipment moves between staff and has to survive established cleaning protocols.
One Platform From Routine Care to Decontamination
Hospitals that receive contaminated casualties need CBRN-capable respiratory protection, but rarely enough to justify a separate system standing idle.
Sentinel XL CBRN Conversion Kit
SKU S-5000-05
Converts a Sentinel HP system to support CBRN and hospital decontamination operations.
In the kit
- S-2001 butyl hood assembly with breathing tube
- Three S-2016 CBRN cartridges
- CBRN CAP 2 user's manual
This is the practical argument for standardizing on one platform. The same blowers, batteries, chargers and belts serve the emergency department on an ordinary day and the decontamination line on the worst one, which keeps spares common and inventory shallow.
Decontamination teams that need dedicated units can choose a complete Sentinel XL CBRN system, the only NIOSH-approved Cap 2 loose-fitting CBRN PAPR. The Standard System, with an alkaline battery or NiMH battery, is packed in a duffel bag. The Backpack System, with an alkaline battery or NiMH battery, uses the NIOSH-approved Sentinel Backpack (S-2008), which supports the blower and cartridges while the CBRN hood is worn and stores the components between uses.

Frequently Asked Questions
What does a hospital need to assemble a complete Sentinel PAPR configuration?
A blower, a battery pack, cartridges appropriate to the hazard, a head top and a breathing tube. The complete HP and CBRN systems on this page include all five, and the Sentinel XL HP Base Kit supplies everything except the head top and cartridges, which a conversion kit adds.
Should a hospital buy complete systems or a base kit with conversion kits?
A complete system suits a department that works in one configuration, such as the HP Standard System with a head cover or the HP Clear Hood and BioShield systems. The HP Base Kit suits departments that need more than one configuration: it supplies the blower, NiMH battery, charger, flow meter, breathing tube, belt and duffel bag, and each conversion kit adds the head top, breathing tube and cartridges for one configuration.
Which Sentinel configuration is used for hazardous drugs under USP 800?
The BioShield Full Hood HD Conversion Kit (S-3000-31) configures a compatible Sentinel XL system for hazardous drug protection with a BioShield full hood, breathing tube and OV/AG/HE cartridges. The hood provides APF 1,000 and requires no fit testing. Confirm the configuration against your facility's hazardous drug risk assessment.
Which cartridge should a hospital use: HEPA, OV/AG/HE or CBRN?
Use HEPA P100 cartridges (S-4002) for particulates and biological aerosols with no gas or vapor hazard. Use OV/AG/HE cartridges (S-4012) where organic vapors or acid gases are also present, including the hazardous drug configuration and sporicidal disinfectants based on peracetic acid or hydrogen peroxide. Use CBRN cartridges (S-2016) only with the CBRN butyl hood for decontamination. No Sentinel cartridge supplies oxygen, and the S-4012 is not intended for ammonia or formaldehyde.
How long does a Sentinel XL run on one battery?
The rechargeable NiMH pack (S-2003) is designed for run times of up to 8 hours. The alkaline pack (S-4013) supplies up to 10 hours and comes with five Duracell D-cell batteries. Many facilities hold both, using rechargeables for the working fleet and alkaline packs for reserve units.
Can the battery be charged while it is still in the blower?
Yes. The Sentinel XL Smart Battery Charger (S-2009) interfaces with the rechargeable NiMH battery pack and allows the battery to remain in the blower while charging.
What is the Sentinel Flow Meter used for?
It verifies that a blower is delivering its intended airflow before the unit is put into service. The S-2010 is an easy to read meter compatible with Sentinel XL and XT blowers, and it supports the routine pre-use checks expected of a respiratory protection program.
Is the breathing tube latex free?
Yes. The S-3508 is made from thermoplastic polyurethane and is latex free and hypoallergenic. It extends from 19 inches compressed to 48 inches fully extended in a single size, lays flat, and flexes with the wearer during demanding work.
Can one PAPR platform cover infectious disease, hazardous drugs and decontamination?
Yes. The same Sentinel XL blowers, batteries, chargers and belts serve all three. Infectious disease configurations use a head cover, clear hood or BioShield hood with HEPA cartridges. The HD Conversion Kit adds a BioShield hood with OV/AG/HE cartridges for hazardous drugs. The Sentinel XL CBRN Conversion Kit converts a Sentinel HP system for decontamination with a butyl hood assembly, three CBRN cartridges and the CBRN CAP 2 user's manual.
Which Sentinel items are reusable and which are consumables?
Blowers, batteries, chargers, flow meters, waist belts and breathing tubes are durable items retained across users. Head covers, hoods and cartridges are consumables with finite service life and need planned replenishment, which is why they are sold in multi-packs, including 25-packs of head covers and BioShield hoods and a 30-pack of clear hoods for central supply.
